Doctor Lio was the doctor on staff when the Autobots and Sparky Witwicky brought in Sparky's son Spike, who had been wounded by a stray bullet in a previous three-way battle between the Autobots, the Decepticons, and a human militia. The Autobots laid down cover fire while Lio and the hospital staff brought Spike in. Lio had to hold a concerned Sparky back as they brought Spike in for surgery. During the battle that followed, the Decepticons' leader spitefully opened fire on the hospital, damaging its generator.
As the dust settled, Lio regretfully explained to Sparky and Optimus Prime that without the generator, Spike and many of the hospital's other patients would die. Optimus then used an alien talisman to restore power to the hospital, and an amazed and relieved Lio wasted no time carting Spike off to Intensive Care. Transformers #4
Sometime later, Dr. Lio was present when Spike awoke from his coma, asking where his father was. Transformers #6 As Spike recovered from his wounds, Lio informed him that the bullet had damaged his spine, but that the odds of his walking again weren't necessarily zero. It was at that moment that the hospital came under attack from Astrotrain, who threatened to destroy the hospital if they didn't hand over Spike. Over Lio's protests, Spike willingly handed himself over, and Astrotrain flew off. Transformers #9
Much later, Lio joined the Farmingham militia in forming a convoy to locate the Autobots, now on the run after their base was transported away from them. On finding them, Lio explained that the humans wanted to repay the Autobots for all the lives they saved at Farmingham. And so the Autobots and their new human allies transported the wounded Optimus Prime to the remains of Farmingham Hospital, giving them a place to hide and recuperate. Lio then showed Optimus the remains of another Autobot that had been taken to Farmingham after being recovered from the ocean.
Shortly afterward, Spike's attempt to sacrifice himself to save the dying Optimus Prime resulted in both of them being knocked in a coma, with Spike's hand being stuck in the Matrix of Leadership and Optimus stuck holding Spike's hand. Wheeljack and Lio were unable to free them from one another, but Lio noted that Spike's vital signs were stable. Unsure of what else to do, the Autobots and their human allies transported Prime and Spike to Chicago, where the US military was preparing to make its last stand against the marauding Decepticons.
