Dedicated Decepticon medics are quite uncommon. This unnamed Decepticon may have been a Surgical Engineer or battlefield corpsman. He was seen performing routine repairs and one extensive reconstruction.
It is unclear what the Decepticon medic transforms into, though it may be worth noting that 4 other Transformers also seemingly absent when the Autobots and Decepticons awoke on Earth all had altmodes that were very small.

The repairs on Skywarp were almost complete by the time Megatron came to the medical bay for a damage report on his injured soldiers.
