Basic syntax: 

{{fromthe|NAME|DESCRIPTION|CONTINUITY FAMILY|PORTION}}

gives

NAME is a DESCRIPTION from the PORTION of the CONTINUITY FAMILY continuity family.

Full syntax:

|name=NAME
|description=DESCRIPTION
|portion=PORTION
|continuity family=CONTINUITY FAMILY}}

also gives

NAME is a DESCRIPTION from the PORTION of the CONTINUITY FAMILY continuity family.
