Scalpel's assistants are equipment from the Revenge of the Fallen portion of the live-action film series continuity family.

Scalpel's assistants are wiry, creepy little robots that help Scalpel follow the orders of Megatron.

Revenge of the Fallen

While Scalpel was examining Sam Witwicky, two robots reminiscent of the Insecticons flew over Sam, dropping a mechanical horror onto him. Scalpel ordered it to go into Sam. The robot entered the mouth, put two tentacles out through Sam's nose, and came out through his mouth. Scalpel appears to have killed the robot as he finished his work. Revenge of the Fallen
