Two-Headed Shockwave is a man-made Transformer who resembles Shockwave, but with an extra head. The Shockwaves were persumably created by the remains of the other Shockwave that had his head ripped of by Optimus Prime

Age of Extinction film
Several Two-Headed Shockwave drones were built by KSI. After Galvatron took over KSI, the drones were among those he sent to capture the Seed in Hong Kong in order to cyberform the Earth. Hound shot one of them in the face with his cy-gar, and another is taken out when Optimus sets off Lockdown's grenade to force the Decepticons to retreat. To bad prime missed the opportunity to force the shockwaves to "Give their faces to him".
